Pathogenesis of Initiaton, Active and Chronic phases
(Experimental and Laboratory Research in Rattus norvegigus)
Rheumatoid arthritis (RA) is a systemic autoimmune disease of still-unknown etiology that leads to progressive destruction of cartilage and bone in the affected joints. RA is characterized by proliferation of synovial tissue and accumulation of inflammatory cells in inflammed joints. Synovial cells are markedly activated by cytokines and adhesion molecules, resulting hyperplasia of the synovial tissue. However, the role of adhesion molecules in active inflammatory processes in RA remains unclear.
The aim of this study was to explore the adhesion molecules ( ICAM-1, VCAM-1 and L-selectin), pro-inflammatory cytokines (TNF-a and IL-1) and anti-inflammatory cytokine (IL-10) in pathogenesis of rheumatoid arthritis. This study was performed on Rattus norvegicus as an animal model of RA called collagen-induced arthritis (CIA), by immunization with type II collagen emulsified in complete Freund's adjuvant. Samples were taken from the animals hind paw for paraffin sections. The haematoxilin eosin staining were applied on the section to confirm the histopathologic appearance of the rheumathoid arthritis phases. Indirect immunohistochemical staining method was used to determine the expression of the adhesion molecules (ICAM-1, VCAM-1 and L-selectin). Proinflammatory cytokines (TNF-cc and IL-1) and anti-inflammatory cytokine (IL-10).
Histologically, the joint tissues showed confirmed appearance with the phases of RA. In laboratory test for rheumatoid factor (RF) showed seronegative titer in all group. Using Manova, Anova and LSD test, it was found that there was significant difference between adhesion molecules ( ICAM-1, VCAM-1 and L-selectin) expressed on synovial tissue of RA in compared to control group. It seem to be predominate in active phase than initiation and chronic phases of RA. The expression of cell producing pro-inflammatory cytokines (TNF-a and IL-1) showed significant difference between RA and control group, and predominate in active phase. The expression of cells producing anti-inflammatory cytokine (IL-10) showed significant difference on RA in compared with control group, and predominate in chronic phase.
Based on these results, it is suggested that adhesion molecules (ICAM-1, VCAM-1, L-selectin) and pro-inflammatory cytokines (TNF-a and IL-1) may indicate there relative importance in mediating active inflammation within joint, whereas pro-inflammatory cytokine TNF-a and anti-inflammatory cytokine IL-10 play a role in chronic phase.
